Module 2 Overview Introduction Becoming aware of specific factors that impact the health of a community is one of the first steps taken before developing a program to address the health related needs of a community. This awareness starts with riding, driving, or walking around in a specific community of interest. A community assessment is a detailed description of a specific community and the people who live in that community. This process is called a windshield survey, which is done by riding and/or walking through a community and using all of your senses. A windshield survey is one component of conducting a community assessment. Various community assessment frameworks can be used to evaluate the strengths and deficits within a community. Many people view and define communities based on what is missing or not available in the community. A framework that is often used by the public health sector is the community assets model, however, there are many other frameworks that can be used to assess the health of a community. As you move about in your personal community take notice of things that might have an impact of the health of people in your area. Advertisements on billboards and signs, even graffiti can have an impact on the health of a community. You may be surprised to learn that many things, which on the surface do not appear to play a role in the health of a community, may significantly impact the health of a community. For example, communities that have a significant number of accessible bike and walking paths have a lower prevalence of obesity. Learning Objectives After completing this module, you will be able to: •Begin to integrate findings from the Windshield Survey of Sentinel City with your target population •Identify a community assessment model that can be applied to your target population •Develop a community health nursing diagnosis for a vulnerable population affiliated with your practice learning site that can be applied to the same population in Sentinel City. Reading & Resources •Harkness & DeMarco (2016) Read Chapters 5, 9, & 11 •Review the Module 2 Lecture. •Download Sentinel City Demographic Database (file included in this module). You will be using this database in Discussion 2 – Question 2 and for the Final Project. •Visit Community Health Nursing Flashcards •Read Community Assessment Models and Measures •Visit “All Health Topics” and click on your health topic •Read the Practice Experience Guide located at the top of the course in the Practice Experience section •Review the Data Comparison Table (included in this file). This table will be used for Part III of your Final Project. Learning Activities •Discussion: Participate in Discussion 2 – Questions 1 & 2. •Your Practice Experience Request is due this week. Read the BSN Quick Start Guide found under Required Documents in the course. Watch the instructional video in the assignment area for the Request for Practice Experiences, and submit your request through the electronic link. Discussion 2 – Question 2 Using demographic information from the 2013 Sentinel City demographic database, as an example, briefly describe the target population that is served by your practice learning site. Discuss at least 2 health status indicators applicable to your target population. Next describe the community assessment model (framework) appropriate for your target population. Include a rationale for selecting this model. What types of information on the CLAS website pertain to the population served by your practice learning site? Include at least 2 scholarly references – 1 reference should be from a peer-reviewed professional journal published within the past 7 years.
